Disabled children - how to access services

An initial assessment will be carried out by the referral team to determine whether or not your child meets the eligibility criteria for a further assessment and services from the Disabled Children's Service Team.

If your child is eligible for an assessment from the Disabled Children's Service Team, a Social Worker will come to your home to meet with you and your child to discuss your family's needs. The Social Worker will provide you with an information pack, which will include a A Guide to Assessment, at the meeting.

Some services, such as summer playschemes and youth clubs can be accessed without an assessment of need. For further details, please phone the SNIX Information Line 01604 236887.

You can contact us directly to arrange an initial assessment. Alternatively, you can ask your GP, Health Visitor, Paediatrician or other professional who knows your child to contact the county council on your behalf.
