Transition planning -
will begin to take place as part of a young person's Special Educational Needs (SEN) Annual Review in Year 9. This is to ensure that young disabled people receive the necessary support to help them plan for their future and to achieve their goals in adult life.
Where a young person is already known to the Disabled Children's Service, a planned "transfer" to specialist support will begin to take place when the young person reaches the age of seventeen.
If you have a Social Worker, please ask them about this. If you do not have a Social Worker please contact the Adult Care Squad.
Connexions Northamptonshire -
is a service for young people aged 13-19 (or up to 25 for young people with special needs) which aims to:
- bring out the full potential of all young people
- help them successfully move in to adult life
- enable them to get the most out of education, training and employment

Moving On Guide -
for young people aged 13+ with learning difficulties or disabilities and their parents and carers.
This guide will help young people with a wide range of abilities, to start looking at what services may be available to them when they are older. Not all of it may apply. The booklet gives young people a number of choices, and will show them where they can find support and more information.
The guide can be downloaded from the Connexions website (see related link below), or a copy can be obtained from a Connexions Personal Advisor or the young person's school.
Northants Special Needs Advocacy project (SNAP) 4 Young People -
is an independent advocacy service provided by Spurgeon's Child Care, for young people between the ages of 13 and 19. It provides a service which is a both a "listening ear" and a "voice" for disabled young people, who can be assured that their privacy will be respected at all times. "Advocacy awareness" can also be provided to groups of young people with special need

Disability Link -
is a service for physically impaired adults in Kettering, Corby, Wellingborough and East Northamptonshire.
The aim of the service is to support disabled people in achieving maximum independence, choice and control in their lives. Referrals are welcome from disabled people themselves, or, with their permission, anyone can refer on their behalf.

Mental Health Services -
Most mental health problems are usually treated by GPs in primary care. They may suggest some form of self help using local resources, perhaps through exercise therapy or finding out more information about your condition.
The GP's will also have a wide range of counselling and advice programmes that they may refer you to. Other support programmes are also available from local voluntary organisations like MIND and self help groups.
People with more critical mental health problems may be referred may be referred by their GP to the local Community Mental Health Team or to other services provided by the Northamptonshire Healthcare Trust in partnership with Northamptonshire County Council.
