  • Adult Care Team
    The main point of contact for all enquiries regarding adult social care should be made through the Adult Care Team unless you have a care manager and their contact details.

  • Community Occupational Therapy
    A single point of access for all referrals.

  • Out of Hours Services
    We know that not all unexpected major problems happen during normal office hours or can wait until the next day. The Out of Hours team is there to help with these emergencies.
