Practitioner Handbook

Version 6 of the Handbook - updated in September 2011 with updated forms, action plan, and separate training slides for the Handbook are available below:

The Common Assessment Framework (CAF) Help Desk would like to announce they will begin a new service for practitioners using the Common Assessment Framework. This service will involve the CAF Help Desk arranging (on behalf of the practitioner who carried out the assessment) the first Team around the Family (TAF) meeting after the CAF assessment.

Practitioners who have completed an assessment will need to fill in and return the new Team Around the Family meeting form, ensuring that they have consent to share the CAF form with the practitioners and teams they list. (See TAF Meeting Form New below).

Please note as the Practitioner you are responsible for inviting the Parents / Carers or Young Person. We would also ask you invite people form your own Organisation.

The Team around the Family Administrators will then arrange the meeting for you. If you require any further information please, ring the CAF Help Desk on 01604 237336, who will continue to be available for advice and support on the CAF process.
