18 to 25 Disabilities Team
The 18 to 25 Disabilities Team work with young people with disabilities as they approach adulthood and leave education. The 18 to 25 Disabilities Team work with colleagues from the Disabled Children’s Services when children are already receiving support from them. All referrals to the 18 to 25 Disabilities Team are through professional staff from Childrens' Services.
If your son or daughter will need social care as an adult, you will need to contact the children’s service if they are still under 18 years old. If they are over 18 you will need to complete the Northamptonshire Adult Social Services Self Referral Form.
If you are a professional or member of the public and have a safeguarding concern about an adult who is vulnerable due to their age, physical needs or mental capacity, then you can either complete NASS’s online form or you can call 0300 126 1000 during office hours.

The Adult Safeguarding website provides local and national information and guidance about keeping people safe who are at risk of harm and abuse.Olympus Care is a part of NASS and provides specific support packages such as Shared Lives, Employment and Disability Services and leisure opportunities at a variety of venues.
