Service description:

Countywide service to deliver the Healthy Child Programme Birth – 5 years via 11 locally based teams
Health visiting teams provide health and development reviews for children at 10-14 days, 6-8 weeks, 8 months and 2.5 years. They also provide advice and support to parents to promote their child’s development and support the uptake of breastfeeding and routine childhood immunisations. Health Visiting teams also provide a key role, in conjunction with others, to safeguard children within the early years period.
Birth to 5 years. All children are offered the routine health and development reviews and parents are offered access to follow up advice and support.
Health Visiting teams would also provide follow up and referrals on for families where there are concerns regarding safeguarding children.
All new births are notified to the teams by the Child Health Admin Teams. Children who transfer into the area are notified to the teams by the GP practice.
